Liability Waiver (Updated AUG 2021)

Agreement with the following items are mandatory in order

to proceed with classes at R Family Dojo during COVID

1.The unvaccinated student will not come to class knowingly having contact

with a COVID positive person, until a 3 day negative test result.
2.The student will not come to class if they have a fever 100.3

or higher.
3.The student will not come to class if they have any signs or symptoms

of COVID, not associated with known allergies, including the following:

Cough, shortness of breath, fatigue, muscle or body aches, headache,

loss of smell, sore throat, nausea or vomiting, diarrhea, pain or

pressure in the chest

4.Students must bring their own water bottle
5.Students may or may not choose to wear face masks

6.If a student is under 18, parents agree to the above terms.

R Family Dojo is NOT responsible if someone contracts COVID and

there is contact tracing to the karate school. We will do our best to

follow all state guidelines and depends on the members to do their

best to follow the above rules.

We will continue to limit class sizes but will be able to provide alternate

classes in the event of longer term absences or vacations.

New VT guidelines have eased restrictions so that we may move forward

with teaching self defense and have some contact. R Family Dojo will

continue to operate under slightly stricter guidelines to keep students

safe.

**ALL R FAMILY DOJO BLACK BELT TEACHERS HAVE BEEN VACCINATED**
